Excess dose of paracetamol (greater than 2000mg per day)causes liver toxicity since the drug primarily metabolised by liver and in the hydroxylation step hepatic enzyme cytochrome P450 enzyme (along with its iso enzymes CYP2E1 and CYP1A2) results into a metabolic product N-acetyl-p-benzo-quinone imine which is quite toxic.Individual toxicity may be due to the third iso enzyme CYP2D6.
